Description

The aim of this project is to learn from the patient experience of participating in the Think Kidneys quality improvement programme; finding out from patients what went well in trying to routinely collect and act on patient reported measures, where the challenges were and how they can be resolved. This will form part of the research evidence base on improving patient participation in care and decision making.

In accordance with the NHSE Five Year Forward View, the aim of the Think Kidneys programme was to develop pathways of care which support kidney patients to be able to make decisions about and manage their own health. Fourteen renal units took part in Think Kidneys, implementing a series of tools and patient outcome measures into routine clinical practice. This was done with varying degrees of success.
