Description
The role is being funded to define/scope the regional space where the theme can implement findings relevant to integrated care & integrated data - that brings in findings & knowledge
from a range of projects, including the frailty project. In the process knowledge (using a robust approach) will be generated about the integrated care/data agenda to support coimplementation of findings and importantly identify opportunities to access data and answer critical questions for frail older people and people at the end of life.
