Developing Antimicrobial Stewardship: From Policy to Sustainable Practice: A Framework for Sustainable Antimicrobial Stewardship in Healthcare Settings

Abstract

Antimicrobial resistance (AMR) poses a critical global health threat, causing more than 1.27 million deaths annually and risking up to 10 million deaths per year by 2050 if left unaddressed. Evidence-based antimicrobial stewardship (AMS) is essential to preserve antibiotic effectiveness, optimise patient outcomes, and reduce resistance.

This presentation outlines practical strategies for sustainable AMS implementation, drawing on WHO AWaRe classification, UKHSA Start Smart – Then Focus, and the South Centre GUIDE Framework. Key concepts include improving diagnostic stewardship, optimising empirical and targeted therapy, de-escalation, multidisciplinary collaboration, and system-wide stewardship integration.

Using real-world challenges and case-based learning, the presentation demonstrates how clinicians can apply structured AMS approaches to enhance patient safety, reduce inappropriate antibiotic use, and support long-term resilience of health systems.
