Abstract
This research presents the development of the SMART-AMS digital model as a strategic framework to strengthen antimicrobial stewardship (AMS) and combat antimicrobial resistance (AMR), aligned with lessons from the COVID-19 pandemic. Conducted across a UK NHS Foundation Trust, the study employed three sequential methods: a systematic literature review (2000–2021) on AMS in acute care, a retrospective analysis of 640 medical records (2019–2020), and a prospective survey of 240 healthcare professionals assessing AMS knowledge and perceptions. Key disruptions during the pandemic included reduced antibiotic reviews (81%), AMS audits (70%), and IV-to-oral switches (67%). The SMART-AMS model integrates a real-time digital dashboard, mobile AMS guides, AI-driven prescribing tools, adaptive education, and continuous monitoring. It provides a scalable, data-driven framework to guide clinical decisions, ensure quality improvement, and maintain AMS during health crises. This innovative model offers a practical pathway to promote sustainable antibiotic use, enhance health system resilience, and advance global AMR mitigation.
