Abstract
Patient counselling is a cornerstone of pharmaceutical care, ensuring safe, effective, and rational use of medicines while fostering a patient-centred approach to healthcare. This presentation outlines the historical evolution and definitions of patient counselling, highlighting the shift from medication-centred practice to patient-centred care. It emphasises the complementary roles of pharmacists and patients, where pharmacists provide tailored information, verify understanding, and motivate adherence, while patients actively engage in monitoring and decision-making regarding their therapy. The session reviews structured steps in counselling, including preparation, establishing rapport, assessing patient knowledge, delivering clear information, and verifying comprehension. Key topics covered in effective counselling include medication purpose, dosing, administration, storage, side effects, interactions, lifestyle modifications, and follow-up monitoring. The presentation also draws on guidelines from the American Society of Health-System Pharmacists (ASHP) and international best practices, underscoring the importance of communication, collaboration, and shared responsibility in optimising patient outcomes. Ultimately, patient counselling is positioned not only as an educational process but also as an interactive partnership that enhances adherence, reduces risks, and improves quality of life.
