Abstract
Chronic diseases, also referred to as noncommunicable diseases (NCDs), represent a leading global health burden, responsible for over 70% of all deaths worldwide. Effective management of these conditions requires multidisciplinary collaboration, with pharmacists playing a pivotal role in improving medication-related outcomes. This presentation defines chronic diseases and differentiates them from acute conditions, reviews their prevalence and major risk factors, and highlights evidence-based approaches for their management. Key models such as Medication Therapy Management (MTM) and Comprehensive Medication Management (CMM) are explored, alongside the emerging role of functional medicine in addressing underlying risk factors. The presentation further examines how pharmacists contribute to chronic disease management through medication reviews, adherence support, patient education, optimisation of pharmacotherapy, and preventive health strategies. Examples from chronic kidney disease and integrated multidisciplinary care illustrate the value of pharmacist-led interventions in improving patient outcomes. By the end of the session, participants will understand the principles of chronic disease counselling and the expanding role of pharmacists as essential members of healthcare teams.
