Abstract

This presentation explored the role of pharmacist-led pharmacotherapy in clinical practice. It highlighted pharmacists’ contributions to therapeutic decision-making, drug dosing, monitoring, and optimisation of treatment protocols. The presentation demonstrated how pharmacists collaborate with multidisciplinary teams, interpret clinical and laboratory data, and apply pharmacokinetic principles to improve patient care. It further examined pharmacists’ involvement in preventing adverse drug reactions, managing chronic diseases, and supporting emergency and critical care interventions. Emphasis was placed on the impact of pharmacist-led interventions in enhancing patient safety, reducing medication errors, and contributing to sustainable healthcare delivery.
